Preface
Warranty Product Returns
Before returning any equipment to Carrier Access, first contact the distributor or dealer from which you
purchased the product.
A Return Material Authorization (RMA) number is required for all equipment returned to Carrier
Access. Call Carrier Access Customer Support at (800) 786-9929 or (303) 442-5455 for RMA number,
repair/warranty information and shipping instructions. Be prepared to provide the following
information:

Carrier Access serial number(s) from the system chassis or circuit card(s)
l
Name of distributor or dealer from which you purchased the product
l
Description of defect
Ensure that returned items are packaged properly:
l
Follow Electrostatic Discharge (ESD) Precautions on
l
Enclose circuit cards in anti-static wrappers
l
Cushion items to prevent damage during shipment
l
If replacing parts, use same packaging to return old parts
l
Ensure that shipping address is correct and clearly visible on outside of shipping container
l
If returning equipment to Carrier Access Corporation, ensure that RMA number is clearly
visible on outside of shipping container
