Reason for altering the determination.
Modification comment determination:
User comment about the determination alteration.
Signature Level #
Information about the signatures at Level 1 or Level 2 in chronological or-
der.
Signature date:
Date and time at which the determination was signed.
Signed by (short name):
Short name of user who signed the determination.
Signed by (full name):
Full name of user who signed the determination.
Signature reason:
Reason selected by user for signature.
