SCE Alter Adv Decision
This parameter setting determines how many shocks (after power up) will be withheld (in favor
of immediately resuming CPR) when a shockable rhythm is present but the Shock Conversion
Estimator's Shock Predictive Index (SPI) indicates that the probability of successful
defibrillation is low.
When set to 1, 2, 3, or 4, the calculated SPI index for the analyzed rhythm is compared to the
pre-configured threshold value. If the SPI value is below the threshold (in the presence of a
shockable rhythm) or the ECG rhythm is non-shockable, then a NO SHOCK ADVISED
message is issued. If the ECG rhythm is shockable and the SPI value is above the threshold,
then a SHOCK ADVISED message is issued. When set to Off, the calculated SPI value is not
used in the Shock/No Shock determination. Only the presence or absence of VF or shockable
VT is considered.
The numerical value of this parameter (1, 2, 3, or 4) determines how many successive ECG
analyses after power up will use SCE results for Shock/ No Shock determination. For example,
setting the parameter to 2 will cause the E Series to use SPI index results for the first two
analyses after power up. All subsequent rhythm analyses will base the shock/no shock
determination only upon the presence or absence of VF or shockable VT. Applying SCE logic
to a number of early advisory analyses emphasizes the performance of CPR when a
shockable rhythm is unlikely to convert to a spontaneous rhythm.
When the E Series is turned off for more than 10 seconds, the unit resets its SCE analysis
counter to zero.
Note:
According to the database used to develop the Shock Conversion Estimator system,
85% of patients received 4 or less therapy shocks. Of those patients that received a
therapy shock, 95% of the patients that converted to an organized rhythm did so with 4
or less therapy shocks.
