the IQM-233 card.
The software version, if calling about IQM-233 Standard or IQS-233
Codeposition software.
A description of the problem.

The exact wording of any error messages received.
1.3.1 Returning Your IQM-233 Card to INFICON
Do not return any component of your IQM-233 card to INFICON without first
speaking with a Customer Support Representative and obtaining a Return Material
Authorization (RMA) number.
Packages delivered to INFICON without an RMA number will be held until the
customer is contacted. This will result in delays in servicing the IQM-233 card.
If returning an IQM-233 card with a crystal sensor, or other component potentially
exposed to process materials, prior to being given an RMA number you will be
required to complete a Declaration Of Contamination (DOC) form. DOC forms must
be approved by INFICON before an RMA number is issued. INFICON may require
that the component be sent to a designated decontamination facility, not to the
factory.
