RETURN AUTHORIZATION
All equipment and/or parts to be returned to Monarch Instrument, for repair or credit, must have a
Monarch Return Merchandise Authorization Number (RMA). This RMA will be issued by Monarch
Instrument Service Department 800-999-3390, or call your local distributor.
When calling for an RMA, please provide your Purchase Order Number and an explanation of why
equipment and/or parts are being returned. After the RMA has been obtained, return equipment
along with the RMA and a copy of the P.O. This will ensure rapid service and/or credit.
C.O.D. CHARGES WILL NOT BE ACCEPTED.
