Card Encoding Form
The Card Encoding Form is provided to organize your standard card data for encoding. To fill
out this form, you will need the following information:
• Your system ID number (on your customized card encoding form)
• Your limitation code table (see
“Limitation Code Field” on page 9
• Your authorization code table (see
“Authorization Code Field” on page 11
• Your price level table (see
“Price Level Field” on page 13
)
• Your restriction code table (see
“Restriction Code Field” on page 14
If you are encoding Manual Entry format cards, you will also need to reference the manual
entry digits table (see
“Manual Entry Card Format” on page 22
). Special format cards (Code,
Supervisor, Manual Entry, Fuel Delivery, Dipstick, Fueler ID, and Fuel Point) have different
layout requirements. See
Filling Out the Card Encoding Form
To fill out the Card Encoding Form, proceed as follows:

Make a photocopy of the Card Encoding Form. Make several copies if you are defining
multiple types of cards. Retain the original Card Encoding Form.

Fill out the information at the top of the form (System Owner, Bill To, and Ship To).
Customer PO # is for your use in assigning an internal purchase order number.

At Card Color: indicate the color of the card that you want.
Note: Consult your Gasboy distributor for a list of available colors.

At Emboss Fields: indicate the fields or reference information that need to be embossed.

At Card Type: fill in the type of card you are encoding by choosing one of the following
designations: SINGLE, DUAL (DRIVER, VEHICLE, and so on), MANUAL ENTRY,
SUPERVISOR.
Note: If you are providing data for dual cards, use a separate sheet for each card type (for
example, one for DRIVER card and one for VEHICLE card). Do not list dual cards on
the same sheet one after the other. Special format cards have different layout
requirements. See,
