Serial number (located on the back of the printer)

Installed options (i.e., interface and host type if applicable to the problem)
•
Configuration printout:
Click
“Print Test Page” in the Configuration Utility’s Printer Configuration tab or reboot the printer
while holding the FEED key on the control panel (see page 20).
•
Is the problem with a new install or an existing printer?
•
Description of the problem (be specific)
•
Good and bad samples that clearly show the problem (faxing or emailing these samples may be
required)
