4. Design your form.
You can start with one of the MTS provided templates and revise it or start
with a blank worksheet and create your own template. For the purpose of
this procedure, assume a simple form created from a blank sheet that will
contain specimen geometry, the data set information, and data from the
middle gauges.
In the following example, the Custom Header and Operator are text fields
that would be typed in as applicable.
