multiple instances of the same adaptor

You need to be especially careful about having multiple instances of the same
adaptor present in the system. For example, if you have an onboard Adaptec 78xx
chipset and then add a 2940UW PCI card then the OS will number the onboard
chipset at adaptor number 0 and the PCI card at number 1. This holds for given
families, so for example, if you had the previous configuration and added in a
Symbios PCI board, it would then, by slha, instance 0 (as there are no other
Symbios boards present already).

noting the bios information

The easiest way to clarify the configuration is to reboot the system a couple of
times so that you can note down the BIOS information as it is printed out.
