Ask your provider these questions about your broadband connection and fill out this ISP checklist so that you will have this
information readily available when you begin your installation.
NOTE: In some cases, you may not need to fill in any information (Example: Most of the Cable Modem Service
Providers). If you think you fall under that category, please skip this table.
