This contamination Disclosure Form (CDF) serves as a means of identifying hazardous materials associated with
equipment being returned to a Celerity facility. Completion of this form supports the regulatory requirements
developed to minimize harmful chemical exposure during transport and final disposition.
Instruction
Please answer all questions completely. If you are unsure of the correct answer, please consult with someone
involved directly with the process. Insufficient or incorrect information could pose an unnecessary risk to anyone
required to handle the equipment. Please complete and fax the CDF back to obtain an RMA number. Any
equipment returned to us without the CDF correctly filled out will be quarantined for further investigation.

Was the system cleaned and purged of all chemical residues? (See purging instructions)

LFC should be cleaned thoroughly with DI water. Ensure valve is in the open position and water flows through the LFC.
Repeat purging three times. Test the water and LFC with Litmus paper, except HF which is tested with a fluoride test
strip (HF is a weak acid and may not show on Litmus paper). Litmus paper should show a pH of 5.0 to 8.5, depending
on acid or base prior use.

MFC/MFM must be cycle purged to ensure removal of all gas contamination. Ensure valve is in the open position during
purging. If residual contamination (gas reacted with moisture in air) is detected, test it with litmus paper. The pH must
be between 5.0 and 8.5 and if not, then purge and clean assembly until a litmus paper test is within limits. If an acid
fluoride gas was in contact with the MFC/MFM then it must also be tested with a fluoride test strip until negative for
fluoride.
•
MegaSys parts must be fully decontaminated by purging with DI water until residual water drained from the part tests
between 5.0 and 8.5 with pH paper. If fluoride was used, then it must also test negative for fluoride using a fluoride test
strip.
Exception: For LFCs only, if the LFC is returned without decontamination for QA assessment, the customer as
shipper must ensure it is packaged, manifested, and shipped according to DOT (U.S. Department of Transportation
regulations CPR 49, part 173 and related sections. Note that certain chemicals can be shipped by ground
transportation only.
