Click [Browse] at the next of the “Input file” field.

Locate the folder that contains the CSV file. Select “CSV (Comma
Separated Values) (*.csv)” in the “Files of type” drop down box,
select the file, and click [Open].

Select the model name that the selected file is originally created
from in the “Machine Type” drop down box.

Click [Browse] at the next of the “Output File” field.
