WARNING:

Failure to follow these warnings and

the instructions could result in serious injury or death.

Assemble product according to manufacturer's instructions

for ANY use mode–bedside sleeper as well as the bassinet

or play yard modes, or both.

FALL AND ENTRAPMENT HAZARD:

Do not use this product when the infant begins to push

up on hands and knees or has reached the height of 25

in (635mm), weight of 15 lbs. (6.8 kgs) or 4 months old,

whichever comes first.
Move your child to another sleeping product when your

child reaches this stage.
Do not allow infant in the play yard when this bassinet is

used with a play yard - remove mattress from the play yard.
When used with the play yard as a bedside sleeper:

• Use only with adult beds that are between 29 and 32

inches from floor to top of adult mattress.

• Never use beds with water mattress.

SUFFOCATION HAZARD:

Infants have suffocated:

• In gaps between an extra padding and

side of the bassinet/cradle, and

• On soft bedding.

Use only the pad provided by

manufacturer. NEVER add a pillow,

comforter, or another mattress for

padding. Never use a water mattress with

this product.

To reduce the risk of

SIDS, pediatricians recommend healthy infants be placed

on their back to sleep, unless otherwise advised by your physician.

• If a sheet is used with the pad, use ony the one provided by the bassinet

or cradle manufacturer or one specifically designed to fit the dimension of the

bassinet or cradle mattress.

• Strings can cause strangulation!

NEVER

place items with a string around a

child's neck such as hood strings or pacifier cords.

NEVER

suspend strings over

bassinet or attach strings to toys.

NEVER

place product near a window where cords from blinds or drapes can

strangle a child.

• Always provide the supervision necessary for the continued safety of your child.

When used for playing,

NEVER

leave child unattended.

NEVER

use plastic shipping bags or other plastic film as mattress covers not

sold and intended for that purpose. They can cause suffocation.

• Use bassinet with only one child at at time.

NEVER

place a child under the bassinet.

•

NEVER

store the bassinet in the playard while in use.

• The bassinet must be fully assembled and installed before using.
